The Agency is the pilot site for the Louisiana Department of Health and Hospitals, Office of Aging and Adult Services, Single Point of Entry Program The objective of the pilot is to model a streamlined access for applicants of Medicaid funded home and community-based and facility-based services for the elderly (age 65 and older) and persons with adult (age 21 and older) onset disabilities The Agency is responsible for intake, assessment, level of care determination and referral for applicants to services The Agency administers this program in seven parishes (counties)
